Sky Zone Waiver

Everyone who is going to jump must have a signed liability waiver. If a jumper is under the age of 18, the parent or legal guardian of the minor must fill out the Sky Zone waiver. Waivers for birthdays and group reservations are only valid the day of the event and are not entered into our system. Once a Sky Zone waiver has been entered into the POS system, jumpers do not need to bring another waiver for a period of 1 year. However, previous jumps at Sky Zone do not ensure that your waiver will be on file.

Electronic Waiver

Sky Zone now offers an electronic waiver which is immediately downloaded to our POS system. The electronic waiver can be used only if 1) you are a participant over the age of 18 and is completing the waiver for yourself or 2) you are the parent or legal guardian of a minor who plans on coming to Sky Zone and are filling out the waiver for him/her.

Click here to fill out the Sky Zone waiver electronically for yourself (must be over 18) or your son/daughter.

If you are over 18 and filling out the waiver for yourself:

1. Fill in information under 'Sign up a New Account.' Press continue.  

2. Complete remaining information fields and click 'Create User.'  

3. 'Membership Created' page will appear. Click on 'Continue.'  

4. 'My Liability Waivers' page will appear. Click the circle next to 'no waiver on file' so that it is filled in.  

5. Click on 'Sign a New Waiver.'  

6. Read release form and assumption of risk. If you agree to the terms, enter your driver's license state and number.  

7. Check the 'I agree to the terms of the above agreement' and click on 'OK.'

If you are creating a waiver for a minor and you are the parent or legal guardian:

1. Create a login for yourself (the parent or legal guardian) by completing steps 1-3 above

2. On 'My Liabilty Waivers' page, make sure the circle next to 'no waiver on file' so that it is filled in. Then, click on 'Add Children.'

3. 'Adding a New Child' page will appear; fill in empty fields and click 'OK.'

4. 'Your Children' page will appear; click 'Continue.'

5. On 'My LIability Waivers' page, make sure the circle next to your child's name is filled in. Then click on 'Sign a New Waiver.'

6. Read the release form. Enter your driver's license state and number. Check the boxes certifying that you are the parent or legal guardian, and that you agree to the terms stated in the release form. Click 'OK.'

7. When the minor visits Sky Zone, a call will be placed to you the parent to verify information before the minor is allowed to jump.

Before and after hours events available daily for private groups. All jumpers must complete and sign a Sky Zone waiver prior to entering the Sky Zone playing field. Anyone under 18 must have their waiver signed appropriately by their parent or legal guardian (no exceptions).
